Did you submit your Child/Teen online registration?

Keep in mind the registration process is considered complete when your registration form, payment (minimum $25.00), and Baptismal certificate are submitted. Please provide a copy of Baptism Certificates for all children/teens in all programs. You may scan and email certificate or mail in a copy to FAITH FORMATION, 5205 Upland Rd. ​Camarillo, CA 93012.
